Every so often, the internet rediscovers the same question. Somebody lines up a photograph of a teenage Elvis Presley next to one from a few years later, points at his nose, and asks whether the King of Rock and Roll quietly had something done. It usually gets treated as an unsolvable mystery. It isn’t. The people closest to him answered it decades ago, on the record, by name — and what they said is far more interesting than the guessing.
Start with the most reliable source there is. Billy Smith was Elvis’s double first cousin, meaning they were related twice over, and he was one of the very few people Elvis kept near him right to the end. He spent the final night of Elvis’s life with him. And Billy has talked about this openly.
According to him, Elvis did not like the width of his nose. In Billy’s words, Elvis “did not like the flare in his nose at that time, how wide it was. So he decided to have it trimmed down.” Billy also says Elvis had caps put on his teeth and had facial blemishes removed, and sums the whole thing up in one plain sentence: “Elvis always tried to improve his looks.”

That is not the only account, either. His bodyguard Sonny West gave a very specific one about a later procedure in 1975, and the detail is what makes it convincing. West said Elvis had it done at the Mid-South Hospital in Memphis — and that Elvis checked in under Sonny’s name, which West only found out about because the bill turned up on his own health insurance card.
West’s description of the result is oddly touching. He could barely see any difference at all. “After he had it done, he brought me into his room and asked if I noticed any difference,” West recalled. “Actually, I didn’t until he told me. He had his eyes done and there is a thin little scar around his ears where he had his face tightened. You can hardly notice it.” Then Elvis, delighted with himself, spent the next while trying to talk Sonny into getting it done too. Sonny was not interested.

Now, a bit of honesty about what is and is not certain. The exact dates and procedures are genuinely muddled. Different people who were around him give different accounts — some say the nose work happened in the 1950s, others during the Army years, others in the 1960s. Elvis never discussed any of it publicly, and there are no medical records in the public domain to settle it. So anyone telling you they know precisely what was done and when is guessing.
What is not in dispute is the broad picture: the people who lived with him say he had cosmetic work, more than once, and that he was quietly vain about it.
Which is worth sitting with for a second. Here was a man voted one of the most attractive human beings of the twentieth century, whose face is still recognized in every country on earth — and he looked in the mirror and thought his nose was too wide. Sonny West could not even spot the difference afterward.
Even Elvis Presley did not think he looked good enough. That may be the most human fact about him.
